Complex Care Team – Joondalup Health Campus, Ramsay Health Care
The Complex Care Team are comprised of intensely passionate individuals who provide the best care and experience for patients living with dementia, improving both the patient journey and outcomes. The team have formed robust relationships with hospital personnel to facilitate early identification of at-risk patients, supporting not only patients, but their families and staff to provide best practice care. Often going ‘above and beyond’ to ensure their patients feel safe and comfortable, the team provide meaningful activities through individualised care, and strongly encourage family to spend quality time with their loved one.
Cardiac Cath Lab – Royal Perth Bentley Group, East Metropolitan Health Service
The Royal Perth Hospital (RPH) Cardiac Catheter Laboratory (CCL) Treat and Return Service (TARS) nursing team treats between 30-50 patients per month and coordinate same day transfer and return of patients from a referring centre to the CCL, for investigation and treatment. The team have developed evidence-based processes and materials to ensure a patient centred approach for the transfer and preparation of patients. Demonstrating flexibility, and building strong partnerships, the team competently collaborate to prioritise clinical need across 4 hospitals ensuring consistent high standards which achieves continuity of care and best outcomes for their patients.
Breast Centre – Sir Charles Gairdner Osborne Park Health Care Group, North Metropolitan Health Service
The nursing team at the Breast Centre, Sir Charles Gairdner Hospital, have specialist knowledge and experience to act as a resource providing evidence-based holistic care to patients journeying through treatment for breast cancer. This is delivered with empathy and compassion whilst empowering the patient to make informed choices. The nursing team collaborate with, and assist, multidisciplinary team members, community services, the patient, and their family to coordinate assessment and diagnosis through patient-orientated, specialised care. Constant communication with patients aids to alleviate anxiety allowing patients to remain calm. Fostering a learning culture, many team members are actively involved in the Breast Interest Nurses Group WA (BINGWA), attending evening and weekend seminars to remain contemporary with current research and willingly share new knowledge with colleagues.
Babbingur Mia, South Coastal Health and Community Services
With a focus on maternal and child health, South Coastal Babbingur Mia (SCBM), a clinical team comprising 90% Aboriginal staff, is committed to delivering comprehensive clinical services, tailored for Aboriginal families, providing responsive culturally safe and holistic care to the Aboriginal community served. In 2023, the team assisted with 29 healthy births and provided a range of services including Antenatal shared care, Child Health Clinic, GP services, and Mental Health Support. Through a comprehensive wrap around approach involving young mothers in their Aboriginal Playgroup, over 300 immunisations were administered along with scheduled and unscheduled child health checks. The care provided by the team has enhanced antenatal figures, first trimester antenatal appointments have increased by 14%, and contributed to improved child health outcomes for local Aboriginal children.
